29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:        a.  At the north side of a residential home, employees access a pump jack scaffold from a second story window, exposing employees to fall hazards.        b.  At the west side of a residential home, employees access a pump jack scaffold from a second story window, exposing employees to fall hazards.

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):        At the west side of the building, employees erected and worked from a pump jack scaffold that was approximately 6 feet from energized power lines.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems were not installed before the scaffold is released for use by employees other than erection/dismantling crews:        a.  At north side of building, an employee worked from a pump jack scaffold without guardrails system at an approximate height of 14 feet, exposing an employee to fall hazards.        b.  At west side of building, an employee worked from a pump jack scaffold without guardrails system at an approximate height of 14 feet, exposing an employee to fall hazards.
