29 CFR 1926.451(d)(1): All suspension scaffold support devices, such as outrigger beams, cornice hooks, parapet clamps, and similar devices, did not rest on surfaces capable of supporting at least 4 times the load imposed on them by the scaffold operating at the rated load of the hoist (or at least 1.5 times the load imposed on them by the scaffold at the stall capacity of the hoist, whichever is greater):  On or about April 4, 2016, at the above addressed jobsite, two employees were operating a suspension scaffold to apply stucco, for which the tiebacks anchors were not capable of supporting the greater of; at least 4 times the load imposed on them by the scaffold operating at the rated load of the hoist (or at least 1.5 times the load imposed on them by the scaffold at the stall capacity of the hoist, whichever is greater), which resulted in the failure of one of the two tieback anchors, and subsequent fall of the scaffold and both of the employees from between the 11th and 12th floor of the building.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  On or about April 4, 2016, at the above addressed jobsite, two employees were operating a suspension scaffold which had not been inspected for visible defects by a competent person before each work shift, and for which the scaffold system was observed to have safety deficiencies; such as but not limited to: a missing front guard rail, and tieback components not capable of supporting the required load, which resulted in the failure of one of the two tieback anchors, and subsequent fall of the scaffold and both of the employees from between the 11th and 12th floor of the building.

29 CFR 1926.451(g)(1)(ii): Each employee on a single-point or two-point adjustable suspension scaffold was not protected by both a personal fall arrest system and a guardrail system:  On or about April 4, 2016, at the above addressed jobsite, two employees were operating a suspension scaffold, for which the front guardrail had been removed/not installed, therefore exposing the employees to a fall hazard.
