29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. On or about 6 April 2016 and at times prior thereto, two employees performing metal roof installation activities on the Floresville High School Gymnasium were both connected to a single anchor point capable of supporting one worker or 5000 pounds, exposing the employees to the hazard of falling approximately 20 feet to the ground below.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level. On or about 6 April 2016 and at times prior thereto, an employee performing metal roof installation activities on the Floresville High School Gymnasium was wearing a personal fall arrest system attached to a rope which provided more than 6 feet of free fall, resulting in the employee falling approximately 20 feet to the ground below.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about 10 March 2016 and at times prior thereto, eight employees were performing metal roof installation activities on the Floresville High School Gymnasium without fall protection, exposing the employees to the hazard of falling approximately 20 feet to the ground below. Port Enterprises was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1008116, citation number 1, item number 1, and was affirmed as a final order on 28 January 2015, with respect to a workplace located at 1103 Cincinnati Avenue, San Antonio, TX 78201.
