29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Tielens Construction project site located at 4020 Hwy 42/57, Sturgeon Bay, WI: On April 7, 2016, employees building an addition to a commercial building were exposed to falls while: a) sheathing the roof while working beyond the left and right edges of the catch platform supported by the RT forklift; b) installing bracing on the bottom chords of the truss system without any form of fall protection.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: Tielens Construction project site located at 4020 Hwy 42/57, Sturgeon Bay, WI: On April 7, 2016, an employee was using an 8' fiberglass step ladder to access the truss system which was approximately 3' above the top of the ladder, which required the employee to stand on the head of the ladder.
