29 CFR 1910.110(e)(4)(iii): Permanent and removable fuel containers were not securely mounted to prevent jarring loose, slipping, or rotating:    On or about April 6, 2016, the employer failed to ensure that the propane tank on the Orange Nissan Datsun 4000 Powered Industrial Vehicle (PIV Serial Number was illegible) was secured.  Both metal latches that were in place over the propane tank installed on this PIV were found in the unlatched/unsecured position.

Written machine specific procedures were done in 1999 .  There was an acquisition of one additional item (gantry crane) in 2011 which was not added to the program.  No documentation of any annual reviews submitted.
