OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that:    On or about March 30, 2016, the employer failed to furnish employment or a place of employment free from recognized hazards that are likely to cause death or serious physical harm to employees when the guarding it had in place on mold injection press #12 failed to adequately protect an employee. The guarding was not effective in that it allowed the employee to be struck between two molds during removal of production pieces. The equipment safety mechanisms in place monitored the position of the ram slides and not the molds themselves allowing for gravitational downward force of the molds.
