The operator doors were not interlocked and were capable of being opened during production.    d) On or about April 7, 2016, the employer failed to ensure that the Puma 200 2SP CNC machine was effectively guarded to prevent employees from the point of operation. The operator doors were not interlocked and were capable of being opened during production.    e) On or about April 7, 2016, the employer failed to ensure that the Mori Seiki CNC machine was effectively guarded to prevent employees from the point of operation. The operator doors were not interlocked and were capable of being opened during production. Additionally, the plexiglass barrier guard was broken and missing pieces.    f) On or about April 7, 2016, the employer failed to ensure that the Milltronics SL6 CNC machine was effectively guarded to prevent employees from the point of operation. The operator doors were not interlocked and were capable of being opened during production.       g) On or about April 7, 2016, the employer failed to ensure that the KBC metal band saw was effectively guarded to prevent employees from the point of operation. The guard was not adjusted to cover the unused portion of the blade.
