29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: (a) On or about April 6, 2016, an employee was painting a building while standing on a pallet supported by a forklift. There were no guardrails installed to prevent falls of approximately 9.0 feet. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.602(c)(1)(viii): A lift truck, equipped with vertical only, or vertical and horizontal controls elevatable with the lifting carriage or forks for lifting personnel, did not have the following precautions to include use of a safety platform firmly secured to the lifting carriage and/or forks: a) On or about April 6, 2016, an employee was painting a building while standing on a pallet supported by a forklift. The pallet was not securely fastened to the forklift. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
