29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about April 7, 2016, at Arctic Express, the employer failed to ensure that the strain relief on a 230 volt 3 phase drop outlet from an electrical disconnect was adequate. Employees use the outlet on a daily basis as a power source for a compressor on the refrigerated truck.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information: On or about April 7, 2016 at Arctic Express, LLC, the employer failed to ensure that employee(s) were trained on the hazard communications program, and the new label elements and safety data sheets (SDS) format. Arctic Express, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(h)(3)(iv), which was contained in OSHA inspection number 1089810, citation number 1, item number 2 and was affirmed as a final order on November 24, 2015, with respect to a workplace located at 1169 Wall Road, Broussard, LA.
