29 CFR 1910.147(d)(4)(i): Lockout or tag out devices were not affixed to each energy isolating device by authorized employees: Employees who performed servicing and maintenance on and around a Trommel "Dirt" Separator were not required to affix lockout or tag out devices to each energy isolating device. During cleaning operations involving the opening of the guard doors, a lock or tag was not affixed to the energy isolating device.

On or about 2-23-16, a recordable injury occurred when an employee received days away from work due to a knee injury while operating a front end loader.
