29 CFR 1926.451(f)(15)(i): A ladder placed against a structure which was not part of the scaffold was not secured against the sideways thrust exerted by the ladder.   On or about April 8, 2016, at the above addressed site, an employee was exposed to a fall of approximately 4 feet when stripping wooden forms from a portable ladder set up on the work level platform of a fabricated frame scaffold while the scaffold was not secured against sideways thrust.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about April 8, 2016, at the above addressed site, an employee exposed to a fall hazard of approximately 15 feet 4 inches when stripping wooden forms while stepping on the top of a concrete block wall and not being protected from falling to the lower level.
