(a) In the shipping and receiving area, on or about April 7, 2016 and at times prior to, the employer exposed employees to struck-by hazards by not adequately training forklift operators with regards to the following: composition of loads to be carried and load stability; load manipulation, stacking, and unstacking; pedestrian traffic in areas where the vehicle will be operated; and, narrow aisles and other restricted places where the vehicle will be operated.
