(a)  In the shipping and receiving area, on or about April 7, 2016 and at times prior to, the employer exposed employees to struck-by hazards by not adequately training forklift operators with regards to the following:  composition of loads to be carried and load stability; load manipulation, stacking, and unstacking; pedestrian traffic in areas where the vehicle will be operated; and, narrow aisles and other restricted places where the vehicle will be operated.

29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval and capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly :    (a)  In the shipping and receiving area, on or about April 7, 2016 and at times prior to, the employer provided a boom attachment with "tweezer" and packing lift attachment to employees for the movement of manufactured countertop slabs without approval from the forklift manufacturer or altering forklift labeling to include the modification.
