29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece was used, and at least annually thereafter: On or about April 8, 2016, employees were being exposed to hexavalent chromium (chromium VI) while being required to wear MSA Advantage (TM) 1/2 face tight-fitting respirators during plating operations and the employer did not ensure that annual fit testing was conducted. There is no abatement certification or documentation required for this item.

29 CFR 1910.134(k)(5): Retraining shall be administered annually, and when the following situations occur: (i) Changes in the workplace or the type of respirator render previous training obsolete; (ii) Inadequacies in the employee's knowledge or use of the respirator indicate that the employee has not retained the requisite understanding or skill; or (iii) Any other situation arises in which retraining appears necessary to ensure safe respirator use. On or about April 8, 2016, employees were being overexposed to hexavalent chromium (chromium VI) while being required to wear MSA Advantage (TM) 1/2 face tight-fitting respirators during plating operations and the employer did not conduct annual training.
