29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Indoor Vehicle Maintenance Area: On or about April 05, 2016, a metal "knockout-type" outlet box, attached to an extension cord, was not mounted in accordance with its listing. The extension cord was plugged into an outlet of the permanent building wiring and the outlet box was used to power a Snap-On cordless drill battery charger. Employees were exposed to an electrical shock hazard.

29 CFR 1910.215(a)(4): Work rests on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: (a) Vehicle maintenance area: On or about April 5, 2016, the work rest on the Dayton bench grinder was not properly adjusted to prevent the work from being jammed between the wheel and work rest which may cause wheel breakage. The work rest measured 1/4-inch away from the wheel on the right side. The AZ Army National Guard - UTES, was previously cited for a violation of this occupational safety and health standard 1910.215(a)(4), which was contained in OSHA inspection number 315436048, citation number 1, item number 1 and was affirmed as a final order on May 19, 2011, with respect to same workplace located at 600 E. Track Rd, Florence, Arizona.
