The quaternary ammonia sanitizer used in the system is corrosive. Employee eyes were exposed to damage from a corrosive liquid due to lack of protective glasses or goggles.

29 CFR 1960.25(a): The Agency did not provide the equipment necessary for inspection personnel to conduct a thorough inspection of the workplace:  (a) On or about April 7, 2016 at Munsee Foods in Muncie, Indiana, it was determined that the USDA personnel in charge of inspecting Munsee Foods did not have sufficient knowledge or competency to conduct a thorough inspection of the facility. Some of the hazards missed included, but were not limited to: leaking anhydrous ammonia valve that was repaired with Teflon tape instead of replacing the valve; lack of an eyewash and drench facilities where corrosives and acids were in use; and a lack of safety and health programs.

There was no safety data sheet (SDS) available for the unlabeled chemical. The USDA employee contended that he could get the SDS, but was directed by management not to provide it. The requested SDS has not been provided.
