29 CFR 1910.178(m)(1): Trucks shall not be driven up to anyone standing in front of a bench or other fixed object. a) On April 7, 2016 a powered industrial vehicle operator drove their lift into an employee, in that while they were in the freezer area in zone F1-115-1A operating powered industrial lift trucks one employee struck another employee pinning them between the Pallet Jack #WR6000 and the (fixed object) Reach Truck #ND3000.

29 CFR 1910.178(n)(3): Other trucks traveling in the same direction at intersections, blind spots, or other dangerous locations shall not be passed. a) On April 7, 2016 in the freezer area zone #F1-115-1A operators of powered industrial lift trucks such as but not limited to the #ND3000 Reach Truck and the #WR6000 Pallet Jack wee exposed to blind spots as the operators went around the corner and to past another employees, exposing employees to struck by hazards.

29 CFR 1910.178(n)(6): The driver shall be required to look in the direction of, and keep a clear view of the path of travel. a) On April 7, 2016 an employee was struck while getting off of a Reach Truck model #ND3000, the operator traveling on a motorized pallet jack Model #WR6000 did not keep a clear view of the area hitting/ striking an employee. The ISupply was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(n)(6), which was contained in OSHA inspection number (1071546), citation number (1), item number (3) and was affirmed as a final order on November 23,2015, with respect to a workplace located at 1255 Spangler Road, Fairborn, OH 45324.
