29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: On or about April 7, 2016, employees working in the Addition were exposed to electrical shock hazards: a. the flexible cord attached to the heater to keep the buckets warm in the Addition was plugged into a triple outlet wall adapter which was plugged into a red extension cord which was run along a steel beam and the wall and plugged into a wall outlet. The flexible cord and extension cord were being used a substitute for fixed wiring. b. the flexible cord attached to an overhead light located in the Storage Area on the Mezzanine was run though the wall and plugged into an extension cord which ran through a walkway and down a flight of stairs and plugged into a wall outlet. The flexible cord and extension cord were being used as a substitute for fixed wiring. c. the flexible cord attached to the CM Lodestar 1/2 ton electric hoist located in the middle of the Main Room was plugged into a yellow extension cord which was run along a cable and plugged into a wall outlet. The flexible cord and extension cord were being used as a substitute for fixed wiring.
