29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: a) Hale Performance Coatings, Inc. located in Toledo, Ohio; On or about April 11, 2016 the employer failed to ensure those employees who were required to wear half-mask elastomeric respirators were clean shaven to allow a good face piece to skin seal. Employees were exposed to hazards associated with lead exposure. An employee working in the lead melt room was exposed to lead at an eight hour time weighted average of 0.044 mg/m3, exceeding the action limit.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Hale Performance Coatings, Inc. located in Toledo, Ohio; On or about April 11, 2016 the employer failed to ensure that the emergency eye wash and shower stations located in the dye clad and chrome building were operational. Employees were exposed to corrosive chemicals such as, but not limited to, chromic acid and sulfuric acids.

Employees were exposed to hazards associated with lead through ingestion.
