29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded:  In the following instances floor holes into which persons can accidently walk were not guarded:  a)    Biddeford - In the basement the hole used for access to the sump pump was not fully enclosed.  b)    Biddeford - In the basement the floor drain between turbidity units 5 and 6 was not covered. c)    Duck-A-Way - The sump pump hole at the bottom of the stairs was not guarded or covered.

29 CFR 1910.27(c)(4): The employer did not install a fixed ladder so that the nearest permanent object in back of the ladder shall be not less than 7 inches, except when unavoidable obstructions are encountered, the ladder shall be installed with clearances as shown in figure D-3.  From figure D-3, the employer did not install the fixed ladder so that there was at least a 4 1/2 inch minimum clearance between the obstruction and the centerline of the rung just beneath the obstruction:  Tidal Brook Well House Pump Station - The fixed ladder providing access to the pump station was not installed so that there was at least a 4.5" minimum clearance distance on the back side of the ladder.
