29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a. On or about April 8, 2016, employees operating nail guns were not wearing safety glasses while installing roofing shingles, being exposed to eye injuries.       Roofing Labor, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1) which was contained in OSHA inspection number 1109311, citation number 01, item number 001 and was affirmed as a final order on February 17, 2016, with respect to a workplace located at 11455 Lowndesboro Drive, Jacksonville, FL 32259.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a. Employees installing roofing shingles at elevations of eight feet or more above the ground are not being protected from fall hazards by a fall protection system.    The employer fails to assure effective protection against falls as required by the standard, as demonstrated on April 8, 2016, when roofing employees installing shingles on a 4:12 pitch roof at a residential structure at 5454 Robert Scott Dr. N., Jacksonville, FL, were exposed to a 8 feet fall hazard above the ground in that they were not using any type of fall protection system.    Roofing Labor, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1109311, citation number 01, item number 002 and was affirmed as a final order on February 17, 20116, with respect to a workplace located at 11455 Lowndesboro Drive, Jacksonville, Florida 32259.
