Employees engaged in insulating the second level were exposed to fall hazards while walking/working.  This condition exposed employees to a fall hazard of 12 feet.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with one handrail and one stairrail system along each unprotected side or edge:       (a)City Wide Insulation @ 46594 W. 268th St., Sioux Falls, SD 57106:  On and prior to April 5, 2016, the exposing employer did not ensure that the main stairway within the building, was equipped with an adequate stairrail system, in that the midrail was not appropriately installed. Employees were accessing the second level bridal suites to blow insulation. The condition exposed employees to a fall hazard of approximately 12 feet.
