29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate respiratory hazards in the workplace to include a reasonable estimate of employee exposure to respiratory hazards and an identification of the contaminant's chemical state and physical form. a) On or about 04/07/2016, B&C Fireproofing & Insulation employees working on the Bed Tower Expansion Project at the Department of Veteran Affairs Medical Center located at 1030 Jefferson Ave in Memphis, TN, exposed to Isolatek International Cafco Blaze-Shield II Spray Applied Fire Resistive Material which contains slag wool, Portland cement, calcium sulfate, and quartz and can cause eye and skin irritation and can be harmful if inhaled and the employer did not estimate or measure employees' exposure to the ingredients of the material used. Employees voluntarily wore N95 filtering facepiece respirators. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
