29 CFR 1910.23(a)(3): Every hatchway and chute floor opening was not guarded by one of the following:  (i)  Hinged floor opening cover of standard strength and construction equipped with standard railings or permanently attached thereto so as to leave only one exposed side. When the opening is not in use, the cover shall be closed or the exposed side shall be guarded at both top and intermediate positions by removable standard railings.  (ii)  A removable railing with toeboard on not more than two sides of the opening and fixed standard railings with toeboards on all other exposed sides. The removable railings shall be kept in place when the opening is not in use. Where operating conditions necessitate the feeding of material into any hatchway or chute opening, protection shall be provided to prevent a person from falling through the opening.  A) Digester building 6th floor, on or about March 31, 2016, employees have to work near chute openings in the floor that are not guarded, exposing employees to a fall hazard of 8 feet.

Section 14-3 Special Purpose Industrial occupancy; did not have exits and other features in accordance with the provisions for general industrial occupancy:   A) Digester Building, on or about April 1, 2016, exit signs were not posted over doorways or access ways to stairways on the 6th floor, 5th floor, and 3rd floor, as required in section 14-2181, that lead to exits, exposing employees to a hazard of burns or chemical exposure from not being able to exit the building quickly in case of a fire or other emergencies.
