29 CFR 1910.147(c)(6)(ii) The employer did not certify that periodic (annual) inspections of energy control procedures were performed. The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. On or about April 5, 2016 at the Architectural Design Concepts facility - In that an examination of program elements determined that periodic inspections of energy control procedures were not accomplished and certified. Specifically, reviews were not conducted to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources. Abatement certification is required for this item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about April 5, 2016 at the Architectural Design Concepts facility - Employees were exposed to amputation hazards from the DeWalt table saw, in that the guard for the blade had been removed and was not covered or guarded to prevent rotating parts or flying chips/sparks from contacting the operator. Abatement certification for this item is not required.
