29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: Location: 252 Hallet Street, Bridgeport, CT 06607 The cast iron pipe was used as an outrigger to support scaffold planks on the tubular welded frame scaffold was not capable of supported at least 4 times the maximum intended load without failure. two employees were standing on while installing a 200 pound lintel over a window. On or about 04/11/2016 a cast iron pipe, used as an outrigger to support scaffold planks on a tubular welded frame scaffold, failed resulting in two employees to fall 7 feet to the ground.
