Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA:   a) 401-475 Keystone Ave, CenterPoint East, Commerce and Trade Park, Pittston, PA - On or about April 4, 2016, an employee was struck by a traveling forklift and as a result was hospitalized overnight at Geisinger Wyoming Valley Medical Center in Wilkes-Barre. The employer failed to report the hospitalization within 24 hours as the report was received on April 20, 2016.                                                                                                            CORRECTED DURING INSPECTION.
