29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  For the facility at 90 W Orange St., Avon Park, FL 33825, as observed on or about 04/12/2016,  employees were authorized to d o maintenance and/or repair documented specific lockout tagout procedures were not available for equipment such as, but not limited to, the following:  a) Shredder - hazardous energy consists of 480 VAC and mechanical moving parts b) Conveyor - hazardous energy consists of 480 VAC and mechanical moving parts c) Dryer - hazardous energy consists of 480 VAC and high temperature

Hazardous energy existed in the form of 480 VAC electricity, mechanical moving parts, and high temperature.
