29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power presses: (a) Rocky Mountain Orthodontics, Inc. @ 650 W. Colfax Ave., Denver, CO 80204: On or before November 3, 2015, as part of the injury/accident prevention responsibilities under its safety program, the employer did not ensure that the 5-Ton, JAM, Die Press Machine #81-073, Model GPN505S used at the Formed Tubes & Bands Dept., was properly guarded to safeguard operators from hazards such as those created by the Point-of-Operation during its operating cycles. On November 3, 2015, while operating the Die Press Machine with an unguarded point of operation in the foot pedal mode, an employee experienced an amputation of the right middle finger at the 2nd knuckle.
