Vanderventer Avenue, Saint Louis, MO 63110, on or about 4/14/2016, the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard, by the unexpected collapse of damaged storage racks.  At the warehouse area, storage racks, unknown manufacturer, were severely damaged had not been repaired or replaced. One method of abatement to correct this apparent hazard is to repair/replace all rack columns as required by ANSI MH16.1:2012, paragraph 1.4.9. DISCLAIMER:  a) The employer is not limited to the abatement methods(s) suggested by OSHA.  b) The employer is responsible for selecting and implementing an effective abatement method.
