29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade were not guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed: a) Work Site - On or prior to April 12, 2016, employees operate the metric saw (Alpine Engineered Products, Inc. Centerline Saw, Serial Number: 645D03016-00, Model Number: 645) to perform cuts on pieces of wood. The metric saw (Alpine Engineered Products, Inc. Centerline Saw, Serial Number: 645D03016-00, Model Number: 645) has a Speed Cut Barrier Guard attached to the saw. The space between a piece of wood that is cut by the saw and the guard measured to be approximately 1 inch, exposing employees to caught-in hazards due to the Speed Cut Barrier Guard not being properly adjusted.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) Work Site - On or about April 8, 2016, an employee was operating the metric saw (Alpine Engineered Products, Inc. Centerline Saw, Serial Number: 645D03016-00, Model Number: 645) to perform a cut on a wood board. While performing the cut, the employee cut through his left index finger and cut into his left thumb resulting in a work-related amputation. The employer reported this work-related amputation to OSHA on April 11, 2016.
