29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Clauson Colchester Lathe; On or about April 14, 2016 and at times prior thereto the employer exposed employees to amputation hazards in that employees were allowed to operate the Clauson Colchester Lathe without a chuck guard. b) LeBond Lathe; On or about April 14, 2016 and at times prior thereto the employer exposed employees to amputation hazards in that employees were allowed to operate the Lebond Lathe without a chuck guard. c) Lodge and Shipley Lathe; On or about April 14, 2016 and at times prior thereto the employer exposed employees to amputation hazards in that employees were allowed to operate the Lodge and Shipley Lathe without a chuck guard. d) LehMann Lathe; On or about April 14, 2016 and at times prior thereto the employer exposed employees to amputation hazards in that employees were allowed to operate the LehMann Lathe without a chuck guard. e) Victor Swing Lathe (1); On or about April 14, 2016 and at times prior thereto the employer exposed employees to amputation hazards in that employees were allowed to operate the Victor Swing Lathe (1) without a chuck guard. f) Victor Swing Lathe (2); On or about April 14, 2016 and at times prior thereto the employer exposed employees to amputation hazards in that employees were allowed to operate the Victor Swing Lathe (2) without a chuck guard. g) Summit Lathe; On or about April 14, 2016 and at times prior thereto the employer exposed employees to amputation hazards in that employees were allowed to operate the Summit Lathe without a chuck guard.
