29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 22, 2016, in the machine shop the employer did not provide adequate machine guarding on the lathes, drills and mills. a) Lathe: Kingston, HD 2600 did not have a guard attached to cover the rotating cuff that was bolted onto the rear portion of the stock. b) Mill: Kingston, Comet, Jet, have no point of operation guards in place to protect the user from the rotating drill bit. c) Drill: Sharp and the Carlton have no point of operation guards in place to protect the user from the rotating drill bit.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about January 22, 2016, in the machine shop the employer did not provide adequate machine guarding on a Pacific press-brake. It had missing point of operation guards to protect the employees from lacerations, eye injuries and punctures.

The welding machine positive connection cable was not fully and properly insulated.
