29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Location: 499 Bloomfield Ave., Verona, NJ.    a) There were sections of guardrail missing at the 3rd floor and at the access point to the third floor (2nd level).  Employees working/walking near unprotected edges at the 3rd floor and accessing the 3rd floor working level were exposed to potential fall hazards of up to approximately 20 ft. to the ground below; on or about 4/11/16.

Location: 499 Bloomfield Ave., Verona, NJ.     a) Employees were not protected from falling while walking/working near unprotected wall openings at the site. The employees were exposed to a potential fall of up to approximately 3 stories to the floor below; on or about 4/11/16.

Location: 449 Bloomfield Ave., Verona, NJ.    a) An employee was observed working from a tubular welded scaffold that was not fully planked at the working level.  The employee was exposed to a potential fall hazard of up to approximately 36 ft. to the lower floor; on or about 4/11/16.      KM Construction Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1)(i), which was contained in OSHA inspection number 1102376, citation number 1, item number 2 and was affirmed as a final order on 12/15/15, with respect to a workplace located at 244 US Rt. 46, Fairfield, NJ.
