29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition. The employer is failing to prevent employee exposure to tripping hazards. The condition was most recently observed on April 21, 2016 at the jobsite located 2611 North Bluff, Fulton, Missouri, 65251. The employer did not ensure that all areas of the work center are kept clean and orderly. The following areas are in disorderly and unsanitary condition: 1. The coverall building (southwest corner of jobsite) was filled with paper and debris that created tripping hazards to employee(s) working in the area. The paper and debris covered most of the walking surface and was piled in mounds which reached heights greater than approximately 5 feet in some places. 2. The sorting line building (northwest corner of jobsite) area where deliveries are made and was covered by trash and debris that created a tripping hazard to employee(s) tripping hazard to employee(s) as they enter and exit the door on the north side of the building.

29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. The employer is failing to prevent employee exposure to vehicle struck hazards. The condition was most recently observed on April 21, 2016 at the jobsite located 2611 North Bluff, Fulton, Missouri, 65251. The employer did not ensure that the pad to the north of the sorting line building was kept free of obstructions across or in aisles for employee(s) required to operate powered industrial trucks. The passageway was obstructed by trash and debris that left approximately 5.5 inches (aisle at narrowest point: approximately 53 inches wide; forklift width: approximately 42 inches wide) on either side of the forklift for maneuvering. The condition was such that it required that the operator drive the forklift off the pad and onto the soft ground beside the pad, which resulted in the forklift becoming stuck.

29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through. The employer is failing to prevent employee exposure to trip and fall hazards. The condition was most recently observed on April 21, 2016 at the jobsite located 2611 North Bluff, Fulton, Missouri, 65251. The employer failed to cover a floor whole near fixed machinery exposing employee(s) to a trip and fall hazard inside the sorting line building, where an opening underneath the sorter measured approximately 15 inches (Depth) by 415 inches (Length) by 18 inches (Width).
