29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) North end of process line, East side behind second Star Screen - On April 14, 2016, a relocatable power tap (power strip) was supplying electrical energy to a fan and a radio through a series of extension cords (daisy chained) and a second extension cord supplying electrical energy to a second radio. Relocatable power taps are not listed and labeled for the attachment of additional extension cords.  (b) North end of process line, East side of the elevated platform - On April 14, 2016, several electrical extension cords were attached together (daisy chained) to increase the length of the cord to reach a radio located on the opposite side of the conveyor from the electrical source. Electrical extension cords are not listed and labeled to be connected together in long lengths.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  (a)  North end of process line, East side of the elevated platform - On April 14, 2016 employees were exposed to electrical shock hazards from an orange extension cord that was missing it's grounding conductor (third pin). The cord was daisy chained into a relocatable power tap (power strip) and was supplying electrical power to a radio located behind the second Star Screener.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  (a)  North end of process line, East side of the elevated platform - On April 14, 2016 employees were exposed to electrical shock hazards from multiple extension cords daisy chained to each other as well as a temporary power tap (power strip).  These cords were attached to the guard railing of the work platform and ran across the platform deck and stairs with some showing signs of degradation to the outer insulation jacket. These cords were supplying electrical power to two radios and a pedestal fan.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  (a)   North end of process line, East side of the elevated platform - On April 14, 2016 employees were exposed to electric shock hazards from two electrical cords that had their outer insulation jacket pulled from the attachment plug allowing the tension on the cords to be transmitted to the terminal joints or screws. Both cords  were daisy chained into a relocatable power tap, one of the cords was further daisy chained into several other cords that supplied electrical power to a fan and radio. The second cord ran across the walking surface and supplied electrical power to a second radio.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  (a) Catwalk/stairs behind the second Star Screen - On April 14, 2016 employees working on the North end of the elevated work platform were potentially exposed to electrical shock hazards from an energized switch box with an open knockout. The switch box was mounted on the guard rail directly behind the second Star Screener  at the junction of the work platform and the stairs to the catwalk (right side, descending).
