29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.      On or about April 12, 2016, employees were working inside the basket of a Genie S-45 aerial lift installing metal ores (sheet metal)  without wearing a body belt with lanyard, exposing employee to a fall hazard of 14 feet or more above the concrete surface.
