29 CFR 1926.451(g)(1): The employer did not ensure that each employee on a scaffold more than 10 feet (3.1 m) above a lower level was protected from falling to that lower level.     On or about April 8, 2016 and at times prior to, employees working on a multi story commercial building were exposed to falls of more than 12 feet to the ground below.  A complete guardrail system was not in place to provide adequate fall protection.
