29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section:    a/  Specific, written procedures were not developed or implemented for machinery such as but not limited to the Chesapeake Bagel Bakery Empire Bakery Equipment Inc. Bagel Machine, m/n EMP-2H, s/n 95-250.  One employee received an amputation on or about 04/06/16 while removing dough from the unguarded chains and sprockets belonging to this machine.

Bagel Machine, m/n EMP-2H, s/n 95-250, were unguarded.  An employee cleaning the chain and sprokets received an amputation on or about 04/06/16.
