On or about April 14, 2016, employees were using a portable circular saw to frame the residential apartment complex under construction without eye protection, exposing employees to the hazard of being struck by flying objects.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented: On or about April 14, 2016, two employees were framing the breezeway of the residential apartment complex under construction up to approximately 14 feet high without fall protection, exposing the employees to a fall hazard.
