29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:   On or about April 13, 2016, employees were exposed to fall hazards when the employees did not inspect the job made ladder jack scaffold to insure that all components were properly installed and assembled.  The support board for the right end of the job made ladder jack scaffold failed causing the employees to fall to the ground or to the deck steps.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

On or about April 13, 2016, employees were exposed to fall hazards when the employees designed and constructed a job made ladder jack scaffold whose components did not support the weight of the three employees that were on the scaffold.  The employees were not trained to use fall protection on ladder jack scaffolds.
