29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) CMP Reactors 1-6, North West end of shop - on or about April 21, 2016, the employer developed written energy control procedure CMP Reactors 1-6 for the loading, unloading and cleaning of the machines, however, management did not utilize the procedure for energy control activities performed.  b) GC5, South East end of shop - on or about April 21, 2016, the employer developed a written energy control procedure GC5 for maintenance of the machine, however, management did not utilize the procedure for energy control activities performed.  c) GC6, South East end of shop - on or about April 21, 2016, the employer developed a written energy control procedure GC6 for maintenance of the machine, however, management did not utilize the procedure for energy control activities performed.  d) GC7, South East end of shop - on or about April 21, 2016, the employer developed a written energy control procedure GC7 for maintenance of the machine, however, management did not utilize the procedure for energy control activities performed.  e) Mill Lane, South West end of shop - on or about April 21, 2016, the employer developed a written energy control procedure Mill Lane for maintenance of the machine, however, management did not utilize the procedure for energy control activities performed.  f) OPTO 2, North West end of shop - on or about April 21, 2016, the employer developed a written energy control procedure OPTO 2 for ion source work of the machine, however, management did not utilize the procedure for energy control activities performed.

29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that they were deenergized.  a) 7331 William Ave. Allentown, PA - on or about April 21, 2016, employees performed electrical work on pressure chambers removing and installing heat lamp assemblies, but a qualified person did not use test equipment to test the circuit elements and electrical parts to verify that they were de-energized.
