29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:.    a- 37 1rst Street, Surf City NJ: Two company employees were exposed to fall hazards, in excess of 18 feet to the ground surface below, while performing siding installation from a pump jack scaffold whose one leg was resting on the pitched roof of a buidling's shower house.  Condition nobserved on or about 04/16/16.     NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.    a- 37 1rst Street, Surf City NJ: An employee was exposed to fall hazards, in excess of 18 feet to the ground surface below, while performing siding installation from a pump jack scaffold that was missing end stops and a midrail/screen that extended the full distance of the walking/working surface.  Condition observed on or about 04/16/16.     NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a- 37 1rst Street, Surf City NJ: Two company employees were exposed to fall hazards, in excess of 18 feet to the ground surface below, while performing siding installation from a pump jack scaffold that was missing end stops and a midrail/screen that extended the full distance of the walking/working surface and whose legs were not set on a firm and stable foundation.  These employees had not received scaffold hazard recognition training from the company.  Condition noted on or about 04/16/16.     NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
