OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to caught-between hazards.    a.  Located at the Amazon worksite in Pataskala, Ohio, an employee was performing steel erection activities out of a JLG aerial lift, model number 600S, serial number 10169777, that had no key in the lower controls, thereby exposing the employees to the hazards associated with operating aerial lifts.  On or about April 13, 2016, an employee was trapped between the top guardrail of an aerial lift and a horizontal steel I-beam.  The key was missing from the lift's lower controls leaving personnel unable to readily assist the incapacitated employee.
