29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    In the Manufacturing Plant, 224 Industrial Road, Fitchburg, MA 01420, on 4/19/2016 and at times earlier:    (a) Energy Control Procedures LOTO 1, LOTO A-1 though A6, LOTO B-1 through B-7, LOTO C2 through C8, LOTO E-2 through E-4,  LOTO Tumbler, and Dumpster LOTO as well as LOTO procedures for Stretchers 005, 015, 025, 026, 027, 028, 029, 042, 050, 051, 058, 063, and 065 and the Stretcher Tagfast that were in use did not include a specific statement of the intended use of the procedure or procedure scope.    (b) Energy Control Procedures LOTO E-2 through E-4, that were in use did not include magnitude and types of associated hazardous energies,    (c) Energy Control Procedures LOTO A-1 though A6, LOTO B-1 through B-7, LOTO C2 through C8,that were in use did not include magnitude for pneumatic energy    (d) Energy Control Procedures LOTO A-1 though A6, LOTO B-1 through B-7, LOTO C2 through C8, LOTO E-2 through E-4,  LOTO Tumbler, and Dumpster LOTO that were in use did not include specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure (ECP) at least annually to ensure that the procedure and the requirement of this standard were being followed:    (a) In the manufacturing plant, 224 Industrial Road, Fitchburg, MA 01420, on 4/19/2016 periodic inspections had not been performed at least annually for 38 of 40 ECPs.

29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components thereof.    (a) In the manufacturing area, in the facility at 224 Industrial Road, Fitchburg, MA 01420, on 4/19/2016 and at times earlier employees were not removed from shot feed and clamp area in the guarded area of the 6"-8" Securatie Stretcher/Degator (Stretcher #25) during testing or positioning of machines, equipment or components thereof.
