29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Press #15 Area: A 4 inch x 4 inch metal receptacle was being used as an extension cord to power a turn table conveyor line and a tape machine and was not properly secured to a mounted surface, on or about 5/16/16. b) Labeling Area: A 4 inch x 4 inch metal receptacle was being used as an drop down extension cord to power a turn table conveyor line and was not properly secured to a mounted surface, on or about 5/31/16. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
