29 CFR 1910.178(m)(3): Unauthorized personnel were permitted to ride on industrial trucks: At the time of the inspection, it was found that it was normal practice for employees who were assisting forklift drivers to ride on the forklift. On 4-4-2016 a truck driver helping a forklift driver assemble a load was riding the on the forklift to the banding station. When he stepped off the forklift he was struck by the driver and received serious injuries which required hospitalization. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations
