Employee was working under active overhead stucco work with no head protection, i.e. hard hat exposing him to head injuries from falling debris, tools and or materials. Employees were working on and from tubular welded scaffold with the walking/working platform not being fully planked exposing the employees to fall hazards. Employees were working on and from tubular welded scaffold in excess of two feet above the point of access and was not provided a ladder for direct and safe access to the walking/working platform exposing them to fall hazards as the climbed the scaffold components.
