29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  a:  107 Seguine Ave, Staten Island, NY:  The top working level of the fabricated metal frame scaffold was not fully planked or decked and exposed employees to fall hazard of up to 19 feet 10 inches.    This hazard was observed on 9/22/16.  Exterior Stucco Systems, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 1140904, citation number 1, item number 2 and was affirmed as a final order on June 9, 2016, with respect to a workplace located at 9 Leona Street in Staten Island, NY.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a: 107 Seguine Ave, Staten Island, NY:  Safe scaffold access and egress was not provided for employees who were performing work on the top platform of a fabricated metal frame scaffold, approximately 19 feet 10 inches above the ground.  This hazard was observed on 9/22/16.  Exterior Stucco Systems, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 1140904, citation number 1, item number 3 and was affirmed as a final order on June 9, 2016, with respect to a workplace located at 9 Leona Street in Staten Island, NY.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a: 107 Seguine Ave, Staten Island, NY:  Employees who were performing work on the top platform of a fabricated metal frame scaffold were exposed to fall hazards of up to 19 feet 10 inches and were not protected by an adequate fall protection system.  This hazard was observed on 9/22/16.
