29 CFR 1926.404(a)(2): A grounded conductor was attached to a terminal or lead so as to reverse designated polarity: a) On or about 4/5/16 at the Big Tree Volunteer Fire Station Construction Site, 4112 Big Tree Road, Hamburg, NY: A series of two extension cords supplied 120 volt-ac power to a Powr-Flite Air Mover. The designated polarity of the hot and neutral conductors in the cord were reversed. Neither the male or female end of the cord was original. Both ends had been previously replaced. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: a) On or about 4/5/16 at the temporary power stand in the main hallway of the Big Tree Volunteer Fire Station Construction Site, 4112 Big Tree Road, Hamburg, NY: A series of two extension cords supplied 120 volt-ac power to a Powr-Flite Air Mover . The outer sheath of the first extension cord in the series was frayed at the male end, exposing the conductors. NO ABATEMENT CERTIFICATION REQUIRED
