The employees were installing masonry materials from a supported scaffold located at 56 Blue Heron Trace, Medina, Ohio 44256. The scaffold was approximately 21 feet high with a base measurement of 5 feet. The scaffold was not tied into or braced into the structure. b) On or about April 19, 2016, employees were exposed to being struck by falling materials. The employees were installing masonry materials from a supported scaffold located at 56 Blue Heron Trace, Medina, Ohio 44256, the employee working on the ground under the scaffold was not wearing a protective helmet. c) On or about April 19, 2016, employees were exposed to a fall of approximately 15 feet. The employees were installing masonry materials from a supported scaffold located at 56 Blue Heron Trace, Medina, Ohio 44256, that did not have each working level fully planked. d) On or about April 19, 2016, employees were exposed to a fall of approximately 15 feet. The employees were installing masonry materials from a supported scaffold located at 56 Blue Heron Trace, Medina, Ohio 44256, that did not have mid-rails or side rails installed. e) On or about April 19, 2016, employees were exposed to being struck by falling materials. The employees were installing masonry materials from a supported scaffold located at 56 Blue Heron Trace, Medina, Ohio 44256, that did not have toe boards installed to protect employees from falling objects.
